i don't feel good. i feel like i entered a friendship with someone out of pity for them suffering from a disease that causes them to be in constant pain. what can i do? i genuinely enjoy and appreciate their friendship, but, i just feel bad about it. what can i do?

I'd say get over the bad feelings. If you're actually friends with this guy and genuinely appreciate him, it shouldn't be a problem.

Stop seeing the person as their condition and instead as of who they are. If you can't shake that off you should get away from them for THEIR good. It would be extremely hurtful if they realize a factor in your interactions is pity.
