Apparently what I have (epididymitis) is usually caused by a bacterial infection but all the tests I've had done came back negative for that. The urologist said physical therapy might be an option but its hard to find time with school/work. I've read about people still having pain after surgery so I'd like to avoid it if I can.

They dont normally take whole nut + rest of tubes leadibg to it out. But often both of them are infected and castration is the last thing you want to have as male.

But since it is just one, imho it is worth a shot. You can function normally with just one. The body will even increase the remaining in size so you wont be "less male" with just one. Worth a shot.

Medfag here, that's not normal. You need to get a second opinion from another urologist. If all they did was test for bacteria and send you on your way with a recommendation of physical therapy, they've been grossly incompetent and could actually be sued. You need an MRI of your balls to check for testicular cancer or other tumors. It could be very tiny (not observable from the outside) but if it's in or near the duct it could be causing severe pain. This is not something you can leave alone and assume it will get better, you need to see another specialist.
