It looks like its pretty bad. Note the yellow discoloration on the side of the nail, the infection is pretty deep.

Normally I'd be the DIY type and recommend the same, but in this case, I'd probably recommend going to a doctor.

Can you gt access to medications without prescriptions?

1. Disinfect your toe
2. Use local anesthesia -optional-
3. drain the pus, by either making a small incision over it 2-4 mm. or use a needle, needless to say both have to be sterile.
4. Press and milk all the pus until all what you get is blood.
5. Apply local antibiotic ointment (Tuscidic acid or Tetracycline ointment) 3-4 times a day.
6. Warp it in bandage
