How likely is it to get an STD/STI without sexual contact, but possibly just normal contact (skin to skin, etc)?
Also, this is on my elbow. does it look like a symptom of something? Kind of bumpy, flaky, reddish.

Almost 0% chance on most STD's/STi's
Some "diseases" that are considered "sexually transmitted" are borderline though... like HPV. A lot of people get HPV without even having sex.. but I'd hardly consider it a legit disease that you should worry about. It is almost symptomless, as are many STD's/STI's.

What about possibly open directs coming in contact with your skin? My friend told me he might have syphilis, but that was after we shook hands, hugged, normal friendly contact a few times. I guess he didn't think it could spread like that, but I read that it was possible.

Yes you can get syphilis from shaking hands. If at the same time you shook hands your frind was cumming in your butthole. If you are that worried go do a blood culture from you GP doctor.
