Say "hey I think your family are hoarders" either as a conversation starter or during a lull in the conversation. Then if he denies it explain why you think so. If he gets really defensive then apologize and explain you just wanted to make sure he's alright. If no hangups occur you should have a smooth talk about it and get something resolved.

Hoarding of course is indicative of mental illness but I'd go so far as to say it's not as bad as being outright poor and filthy ... If they have bad spending habits it's really up to him to get out of there asap. You just can't live with people who are stupid with money.
