I've made goal sheets, read books, and tackled depression but I still can't get things done. Atleast I don't want to kill myself anymore

Nobody knows the answer to this. You're better off dropping out now to save yourself time and money. Enroll in a trade school where learning simultaneously gives your paycheck, that way your learning is tied directly to your survival. You'll develop a well paying skill set to get through life on.
In a few years when you've learned how to get shit done no matter what (working in the real world will develop this) you can try again and then succeed at a degree.
Every self- help book in the world can't just materialize motivation in a person.

See if your school offers a peer mentorship program. Having someone else's pressure can give you the push you need.

study a hobby, with the web
figure out something of moderate interest to you, watch a video on youtube about it, do some searching about it to find a forum of people talking about it, read some of the stuff they do, become inspired to pursue doing stuff yourself
after that, apply how you studied the hobby to other subjects you're trying to learn
studying works best when you're trying to find the answers to questions you have to fill in gaps in your knowledge, ask questions, find the answers to them, go slightly off topic if you need to

When you get the urge to jack off, go to the bathroom and puss and then start walking/running outside. Instead of going on the Internet start reading some books or do something useful in your time. Like if there's something to be done around the house you could do that. I'm in the national guard and the takes my mind off of jerking off and going on the Internet that often.
Leave your house and go somewhere in public every day. Like a park or the library
