How do I get over the fear of pissing in a public bathroom?

I know their is a term for it, but I can't piss when someone is standing next to me. I noticed that this has always applied to my sex life, as while I'm extremely turned up and have a raging boner, I can't cum. Help?

I just go to the stall. Tried even holding my breath until the muscles relaxed and it simply doesn't come out. I can't pee without effort even if it's my gf watching.
