Spend the whole day doing chores for her, like cleaning, laundry and washing.

This, so this. She would appreciate it sooo much.
well, the reason you give someone a gift is to show you actually give a fuck about them.
so it has to be something that you clearly put in some effort. that effort can be material (as in, is rare/costs a lot/was hard to get), by paying attention (she might have mentioned that she would want something but that she doesn't NEED it and thus can't justify the expenses like those new fancy earplugs) or by spending time (doing something yourself for her that takes effort, like finally rewire her car so she is able to listen music again or, you know, bake a shitty cake for the sake of it) or give her a voucher to spend time with you (doing something she always wanted like a bungee jump).

Maybe you could write her a letter saying how much she means to you and how you're really happy to be able to celebrate her birthday with her another year.
I guess to a combo gift.
Basically give her you whole day doing stuff for her and give her a cheap sorta gift.

Anything that shows a big effort from you works. The specific advice could rebound since we don't know her like you do (e.g. the anon saying to do her chores all day would be right for majority of women, but some would find this disrespectful, as in "you don't think I do my shit well enough"). We just don't know her to give as good specific advice, but you do, so think hard of how you will DEMONSTRATE THE EFFORT. My wife likes surprises, particularly when I really blindside her.
Instead of buying a useless gift like cloth or whateveryouthinkof.jpeg. Give her an experience or a memory for life, like take her out for dinner and a concert etc. That's my best advice.

This.
My boyfriend was broke as fuck in college and he always gave me great gifts because he built me things. He made me a desk, two cute bedside tables and a dresser. Now he's making me a bed frame as a late Christmas gift. He's slowly replacing all my cheap ikea furniture.
Honestly I love all the things he made for me, and totally use them more than all the other gifts I ever got.
