How do normal people have fulfilling relationships?
What actually happens from the point they see an attractive person to a year of a happy relationship?
I can barely imagine it.

Fulfilling relationships are pretty much like any friendship plus intimacy stuff.

How do they establish that they both want to be more than friends?

It happens very organically, if there was attraction towards each other, they'll acknowledge it right away and drive the point home by being more and more direct with compliments and flirting. Sometimes it's more about aligning life goals, but obviously still need some basic attraction.
Basically establish is a bit too hard word for it, there are no walls or doors, it just happens, or doesn't.

story of my life family to be honest

I feel like a child reading this, I didn't even know flirting and compliments were a part of it. The extent of my social life has only been making male friends and I thought relationships were the same but there was some hidden step that differentiates them from friendships or that two friends of an opposite gender just decide they like each other and suddenly start being intimate.
