You have to be diagnosed with serious depression. Just walking in and saying "Hey, I have depression give me a cocktail of poison so I can die." isn't good enough.

The Swiss doctors will be tried for murder if they didn't go through the checks. They don't give it out willy nilly.

Yes but you need a decleration of some problem you have. You can even have Assisted Suicide if you have tinnitus, but you must prove that you do actually have it. In his case, he must prove he has severe depression if he wants to die.
