I went to a therapist, and he had this sign in his office directly facing me the whole time we were talking. What's up with that?
It's a joke sign, but it seems really unprofessional to post something like that.

He switches the sign out for each client according to what he thinks is wrong with them, and he wanted you to notice it and feel bad.

What does that have to do with this?
Probably half joke half for people to think about. Even for people who actively seek help, it is often extremely difficult to adjust their views or the way they do things. The judgemental person in the joke is using his familiar psychological narrative/tactic to deflect the criticism. Another person might have issues with opening up, and keep the therapist at such a distance that they can't actually help the client. Stuff like that.
Basically a reminder that you're not supposed to like or recognize yourself immediately in what the therapist tells you.
