Most likely answer - you are probably not in immediate danger of a heart attack, unless you have some pre-existing heart defect. But due to all the weight gain, you ARE putting a lot more strain on your heart, and you're feeling it.
I'm guessing you're experiencing some general tightness/weird discomfort in the chest area, maybe some occasional circulation issues in your limbs? Basically, your heart has to work a hell of a lot harder now to move your fat ass around, and you're feeling some exhaustion.
You're not gonna drop dead tomorrow, but it goes without saying that this is not good, 34 is not that young anymore, and if your habits don't change you'll be facing some very serious heart problems within the next 6 to 8 years or so.
Go to the doctor just to get yourself checked, see if he recommends precautions like a daily aspirin as a blood thinner, and then get yourself started on an exercise regimen. Laziness will kill you. You're too old for excuses, it's not just about looks anymore.
