Well guro covers a large area, but I'll assume you mean this specific type of guro. To me I think pain has a beauty to itself, and there are some very aesthetic/symbolic qualities to stuff like blood or violence and especially death which is pervasive in the human psyche. Guro is just taking elements already present in traditional art and recasting them in a light of eroticism. I don't see it as a large leap to see pain in an erotic light, nor violence, nor even death. Though most people don't find these pleasing combinations they tend to be interwoven in real life, or at least be an ever present undercurrent.

That said I personally enjoy guro on a more aesthetic level, and I don't really go out of my way to fap to it. So it might be different for people who will go crazy over any depiction of anime gore.
