I think Serpico and Guts both know he needs to get it out of his system. Guts doesn't care, but Serpico really does, and only by being beaten by Guts can he accept it and move on. He pretty much goes right back to helping Guts after the fight, so it was basically to clear his head.

His Dragon Slayer became a cursed weapon from killing so many people that it has an actual astral presence (and can thus hurt Ganishka's astral body), but I think Guts is also being changed by all the shit he's killing as a result.
