It won practically every manga/comic award ever and even got a trilogy of live-action films.
It's probably not more popular because of a couple reasons, like how it fell apart in the final arc and how a lot of what was driving the story forward was nostalgia about a specific era in Japan.

To add to my post, 20th Century Boys is fucking Anno "The Master" tier in that it has so much time spent on what it was like in the past of the kids when you just don't give a shit and it isn't important. It's a lot of hubbub about nothing. "Cool" visuals/art/ideas but the ending sucked, ESP girl was random and it was about some fucking kid being butthurt because people liked his comics and not him since he had a superiority complex and was a big bitch.
Magically he and others gained the power to act out the fantasy of the kid he envied just because.
A lot of the "badass" shit was just bad. Kenji should have just been shot. ESP girl should have just been shot. Plot armor saves them, not even any powers.
Kenji was so fucking contrived when he became "cool." Cliche poses with dialog the equivalent of "Pshh, nothing personal kid." "You don't interrupt a man's guitar playing." [generic downcast looks that hides his eyes to show he's serious] followed by [goofy to salvage realism]
Basically a lot of the shit that always shows up in One Page Threads. Some if it is more well known, like One-Punch Man, and some if it is pretty well known at least on /a/, like hinamatsuri or all of dowman seiman's shit.
I don't think it needs an adaptation, I just feel like a reason why I felt it might have not been particularly well known/relevant in certain circles was down to its lack of an anime adaptation. I guess I might've guessed wrongly that it wasn't well known

Ai-ren was great because of the weight of death in every single chapter.
And Bitter Virgin was teenagers being teenagers with the topic of rape in an actual serious way rather than being used for shock value. And the ending is bittersweet.

Ending was unsatisfying as fuck, but then again all of Naoki Urasawa's works have a shit ending

Urasawa has a trademark storytelling, which can be recognised in pretty much all his series.
Sometimes he does it right, but he did it really bad in 20CB.
As for characters trying to be cool, for example look at kenji and pretty much everything he does after he reappears. Look at what'shisface-longhair, look at Kenji's girlfriend.

The 10 manga bit was obviouslt an exageration. I understand how much shit there is in the medium, but that makes 20CB only better by comparison. If we were to take it as a standalone series, comparing it to nothing but itself then you can see its flaws.

Yes, however you have to admit it's decent up till the first time-skip. Basically till Kana becomes the MC. After that, it only gets worse.
Up till there the mystery was fairly well and interestingly constructed and the characters weren't turned into "good guys who try to play the Hero all along".

I just finished it, and it was a blast. Last arc's story was pretty bad, but it was still enjoyable. MC tried to hard to be cool for no reason when he came back, but I'd be lying if it wasn't based as fuck. 8/10.

I liked 20CB better than Monster but the quality of the series was too variable. It started great then it went to shit, specially early-mid post time skip then it only started to get good again almost in the end

Another manga that I would love to see animated would be Gon, but it would never fly as it is too short and it would be very hard to capture the feeling the manga gives
