I need some tech help. My computer has been freezing for some

I need some tech help. My computer has been freezing for some reason, I've narrowed down a few things, but all the suggestions I've googled don't seem to help.

-All freezes are temporary, lasting anywhere from a few seconds to sometimes a crazy 10 minutes. The average is one minute to a minute and half.
-Freezes only start on a warm boot. Once something causes a freeze, my computer won't do it again until I shut down.
-Skype calls still keep working fine, but the skype screen is frozen. Mouse still moves but can't click on anything
-So far, the things that freeze my computer are Imgur, Nerfnow, and League of Legends. 4chan, other comics, and other games seem to be fine, I'll test more as needed.
-Problem has been going on for a while, tried a few things and some helped, some did nothing, ultimately probably was still there.

Currently using a Radeon HD 5500, AMD Phentom II X2 555 Processor 3.21GHz, with 4 gigs of ram. Not sure if that helps.

Restarted and opened up imgur, small freeze this time, 20 seconds. Firefox and svchost are the only things over 30,000k memory usage.

No. I've tried Warmframe, World of Tanks, and TF2 for starters. Those games all work fine without having the initial freeze that League of Legends does.

Avast and malware bytes. Running another scan now for good measure.

Have you tried upgrading your graphics driver?
I've had issues with my own and it crashing, with similar effects.

Your hard drive is shitting itself. Find whats eating up all you I/O or use a hard drive health checking utility.

No malware, avast detected some junk files but no other problems.

5500 series had its last upgrade in Nov. Fully up to date and no longer receiving updates.

Why would the hard drive be shitting itself when opening only three completely random programs? Would a hard drive problem be caused when it accesses internal memory, not when it is trying to connect to a server?

Well, my graphics driver is updated, too, and it crashes like a motherfucker every other day. (Mostly when I'm using a web browser, Photoshop, and LoL, myself)
I also mostly got this shit when upgrading to Windows 10.
Did you do that? (When I looked into it, it seems other Windows OS's got the same problem- hm.)

I had ended up with a computer with Windows 8 on it- I wanted to change it ASAP so ye I did even if it's a buggy shit

Did a clean up with avast, to a while. Cleaned out a lot of old info from Steam games I deleted but didn't clean up.

Restarted and opened up imgur. Freeze was for about 10 seconds this time. CPU hit 50% from the Resource Monitor, then dropped after the freeze.

I'll restart again and see what happens with the
memory and disk.
