Vanilla has the benefit of being hard to fuck up. I've seen some brands of ice cream really fuck over their chocolate. Usually the biggest problem is texture (ie: too gritty), sometimes its too bittersweet. Strawberry has similar issues of quality.
Plus you can toss jam or stuff on top of vanilla for instant [flavor] ice cream without having to stock up on a billion different pints.

To put it in the most simple terms? It's to make self-harm 'cute'.

I don't think there was a concrete starting point, but rather multiple things. This goes both for Frisk and Chara by the way, just significantly more for Chara. Suffering, especially emotional suffering can be hard to convey, so a lot of artist who either have problems of their own or have no idea of the gravity of what they're doing, go for a great way to show emotional harm, by projecting it as active self-harm.

However, there's this bizarre idea on tumblr especially, that people who suffer can't be bad, ever. And since the other half is some wish fulfillment, Chara must be in deep emotional pain, but also happy and supported. The easiest way to do that? Contrast the bad(scars especially) with something cutesy, and that's how you end up with dozens of Chara's fucking littered with cuts, but patched up with colorful bandages because 'so brave uwu'.
