Twist is that Chaldea does cruel inhuman experiments and Mashu is the last living test tube baby that Chaldea tried to make in secret, when they were trying to basically make an army of living servants. The servant inside of her was put into her ten years ago, she's lying about it being a recent thing.

She's a homu with a set lifespan, since she was made to be relatively disposable. She's set to die at the end of the year. Chaldea used to do a lot of human experimentation. Most recent chapter shows how they kept importing orphans that they could experiment on in secret, and they never disposed of the bodies. So they're just infinitely kept in this near death state.
