Fully built formas have a chance to spawn out of rare and reinforced void crates. Rare ones have a sort of glow like corrupted enemies in fissure missions, and reinforced ones look extra fancy. I think they can spawn in any void related tileset (Lua, Derelict, and regular). They dont have a high spawn rate to begin with, and even then Forma is guaranteed, but if you really wanted to try and farm them, you could basically run Lua missions and how you get the simon says puzzle, since it spawns a rare container every time you complete it.
