Not really required.
Get Ultimate combat, Mortal Enemies and either Wildcat/Combat Evolved/Vigor. They all do the same things essentially.
If you really want it to be hard then get Immersive creatures and return of the enemies.
If you REALLY want it hard, get morrowloot ultimate or morrowloot ultimate redux, scarcity, trade and barter (Use scarcity's recommended settings) and the secret of steel.
Now you have to play the majority of the game in leather/steel until you manage to kill some thalmor jew and steal his elf armor or some orc and get his stuff.
The game becomes much more scrounger like and you often have to work with the shit you pick up which isn't much.
Also forget buying shit because it's all too expensive.

Once you have enough mods, the game becomes extremely hard to balance properly - especially if youre running things like Vigilant, DarkenD, Inferno, and Colorful Magic - shit can just literally oneshot you for the most part.

I play on adept, because at least then I know what the fuck the base level of gameplay is. I usually turn it up around Level 15 or so, because at that point I start to become an unstoppable powerhouse of murder.

Right now my followers have glass-cannon syndrome. I'm looking to extend thier durability and reduce thier damage output, without affecting me or enemies.

I'm not seeing such ability in skytweak. Can it be done?
