I don't think the anime's Medusa was much of an Alter, she's just sort of in between Rider Medusa and Gorgon in Medusa's personal timeline. At least as we saw her in the anime.

Karna is 1/8/10 and Lancer Alter is 8/10/7, 2 level 90s of a class is pretty enough for farming class advantage events. I doubt putting Enkidu on the front line alongside Karna/Lancer Alter is more effective than putting Mashu/Waver with a 2030 in. Heck I think Lancer Alter alone with Mashu/Waver + double 2030s is probably more effective than swapping one out for Karna anyway if the content is hard.
