The whole point of that scene is literally that sicario is as much of a monster as medellin and doesn't give a fuck about some kids or a woman. His specific reasons are relatively unimportant, what's important is that he considers innocent women and children to be acceptable targets.

Cario makes it plane when he says they sit here and eat dinner while families are murdered under his orders. Then he kills his wife and kids in front of him, lets him react and absorb, before killing him.
