I haven't played BB on account of not owning a PS4, but from what I understand of the setting, which is very little, so correct me on any misconceptions, I feel the easiest way to make this autismal crossover of MLP BB and incest fanfiction work would be to substitute the Doll NPC in the Hunter's Dream with either Luna or Nightmare Moon for the obvious reason that she has powers over dreams.
She pulls you, her dying blood relative, into the Hunter's Dream, a place she herself can't escape from.
She does the same thing the Doll did and empowers you, making repeated visits after hunts a must, this would play right into incest nicely, as during downtime in the Refuge, you'd expect loneliness to take its toll on Luna, and what with you constantly dying and going through hell, she might take on a more intimate role in nursing and helping you cope in exchange for the human connection, this dynamic isn't healthy, but shit, nothing in BB is.

The other way would be to just make it a long oneshot that plays out like a regular session of the game but with a girl of your choosing as a relative along on the hunt, that would progress something like this: a little setup to get the reader up to speed > some exploration of a choice area with banter/interaction from the Hunters > some action against mobs > some more intimate heart to heart when resting.The calm before the storm, can culminate in sex here > Fighting a boss and dying, because this is Bloodborne and happy endings aren't a thing here OR beating the boss and, if sex wasn't done yet, the hunters do it now in their heightened state of emotion, right in the pool of blood that used to be a beast, because this is Bloodborne and it has a hard-on for blood > end.

But seriously, I might play it yet some time down the line, I often get my consoles once they near their death, this way I can get them on the cheap, after all the hardware/software crap as been worked out and the libraries are vast, sure some niche titles might be hard to find, but I doubt anything that isn't Demon's Souls would be hard to get now that From Software is mainstream.
I'll be honest, getting consoles even a a year or two after release is getting more and more precarious with hardware malfunctions, no decent games in the lineup for years and now new fucking iterations on the same console not even 3 years later.

That's actually an interesting point, but perhaps not for the reason you think. Sure, you will likely die quite a lot playing it for the first time, but the player's deaths are only temporary and respawning is basically canon. The bosses and NPCs do not, however, and following practically every NPC's questline (to use the term loosely) to the end, including those you are trying to help, almost invariably results in them dying or going crazy and turning into a hollow shell of their former self. Even if you succeed in the game, there are ultimately no happy endings, only death.
