My guess would be to hold it up to your ear like listening to a message, or fake a conversation, and tilt your head at the right angle to catch them.

I mostly do in store ones. If you wait to get in line behind them it's easy to get good ass shots. Walk around the store with your phone at your side, use it from time to time. When you see your target, start recording or start your ninja app, keep it at the same side and just orient your body sideways aiming at them from down low while you look at whatever is straight ahead of you.
