Unless there is more than one enemy alive or it's a boss fight you don't suffer damage during those scenes (but you get debuffs and a short penalty/stun after they finish). Nurse BJ - pick "well endowed" perk
Sophie BJ - Lower her inhibition to around 70 and mom is obsessed
Sophie and Mom drunk rape - Player alignment evil (~35) and incest trait for mom (?).
Mom rapes you - No incest trait, Mom is Nympho or obsessive and corruption is very high.
Threesome Mom and Kelly - Kelly is obsessed and very high corruption, Mom is Obsessed/Nympho.
Mom and Kelly also have separate scenes based on nympho and obsessed states.

Basically corruption = intensity of scene, inhibition = better chance of scenes triggering, mind state (nympho/obsessive) = type of scene that plays.
