They're getting paid on a monthly basis just like any employed person. As long as they release content on a timely basis or at the very least post enough progress updates there's really no problem.
It also gives a lot of indepentents the chance to work on games while still having another job as long as their game isn't bringing in enough money.

The only issue with patreon right now is that developers are abusing a lot of things to make sure they keep the money coming (abusing sunken cost feelings to keep patrons paying regardless of them being happy, using paywalls to coerce non-patrons into pledging, outright SELLING things, etc).

Patreon was designed in a way that pledges are more like donations and less like payments. You're supposed to become a patron if you like someone's work and want to support them so that they keep doing it. So you're also supposed to remove your pledge once you're no longer satisfied (but people are not doing this for some reason).
