If you follow the story, things are pretty clear. Originally, there was a girl called Fumiko. Then her mother forced on her the name Mari. As a result, Fumiko developed a second personality called Mari. Eventually Mari grew obsessed with a neet called Komori, because she thought that he had the sort of "freedom" she longed for. As a result, Mari developed a third personality, which took the identity of Komori.
And in this chapter we can see the three personalities together. The original (Fumiko), the one created due to her mother's abuse (Mari) and the one created out of Mari's longing for freedom (Komori).
Komori might self-identify as male, but that doesn't really matter. What matters is that he represents Mari's wish not to be tied down by her crazy mother. To be free to do whatever she wants to do, and be whoever she wants to be.

It does matter because gender transition is the raison d'être of this whole story, but its not an isolated or something exclusively pinned as we've been mostly concerned if Mari switched bodies or has personality disorder. Do you remember that "Komori" couldn't even look at his genitalia or tits in the beginning and got over that to the point of even masturbating while looking in the mirror? Through all of these events "he" has been transitioning "himself", evolving/transforming "his" mindset to the female being (I know its just another of Mari's personality).
Everyting that happens storywise are the surroundings and circumstances/issues of Mari's life, which includes the longing for freedom.

Actually, I think the longing for freedom is the main issue that connects everything else. For instance, I think the take away from the gender reversal is that Mari wished to escape from her life so strongly that she didn't just took on a new personality, but a personality that was the total opposite from who she was. Mari was popular, a good student, a seemingly obedient kid at home, and of course, a girl. While her new personality (Komori) was a male neet without social life. The total opposite in pretty much every sense of the word.
So the gender reversal is relevant, but mainly as a representation of Mari's longing to escape from her shitty life and her mother. She really doesn't want to be Mari anymore. She probably hates it.
