Because the entire joke about the pills is they're blatantly ripping off Tezuka's Marvelous Melmo, maybe the illusion part is lawsuit-dodging bullshit. Or a very liberal definition where appearances (including size and proportions) are affected but the "true" body (stats and injuries) stay the same.
Essentially, any lewd things the pills cause are real.

No, the thing with the underground monster was it kept Touta's arm alive and only consumed it slowly, and his body could detect that it still was alive, which is why it didn't trigger his regeneration. Touta could practically regenerate lost limbs, case and point, Dana's last spar with him.
