I like having both an online and offline list.
Check anichart. Yellowmark the ones I'm currently watching, green the ones I've seen and red the one's I've dropped.
Plus I have an anime folder with all the shows downloaded in there. I just have to open it and check.

I don't. There's plenty of times I've gotten halfway through a series and realized that I've watched it before, or read the manga. But it's all daijoubu, because I enjoy watching anime.
I usually only watch whatever's immediately available at this point of time and season, kind of like flipping shows on a television.

I've only kept track of manga and vns I've read in a poorly formatted spreadsheet. I have a problem of stupidity that makes me horrible at recalling out of the blue things I've watched or read, though if a name is given I can at least remember if I've done so or not. At lot of stuff just goes in one ear and out the other, or into my eyes and out.... wherever.
