If you want speculative analysis, I think it's a matter of a curve of familiarity.

Rin and Saber didn't get to know Shirou until the events of the routes, so they either don't know his past or don't pick up on his feelings about it. Ignorance keeps them from realizing Shirou's problems until late.

Taiga and Sakura are both fairly close to Shirou even before the events of the routes, so they are more aware of his background, feelings or both. Pity and kindness drives them to avoid mentioning or focusing on Shirou's issues.

MItsuzuri is in a middle-ground as a close school friend and club member. She's neither so close to Shirou to be overcome by pity for him, nor so unfamiliar that she'd be unaware of his trouble. As a result, she tries to be a good friend and brings up what she thinks is a problem with him. Whether or not she's aware of just how screwed up he is on the inside, she notices the signs and suggests he ought to take notice of them too.

Her character was pretty mediocre in the VN as far as I remember, but the animu is really fucking selling her.
