Some things are contextual. Our languages aren't 1-to-1. There are also localizations to make characters' personalities make sense. A direct translation might not have the right tone if read in English, so they change the word choice to match the tone and keep the intent the same. It's why you might recognize words you hear but not see that exact word in the subs. This may be what you're picking up on.

Not really. There are occasionally egregious mistranslations, but usually it's just people spouting off about things they don't understand.

There are a couple of things beginners really don't get about translation. First, there is no such thing as a "literal" translation, especially with a high context language like Japanese. I've heard that deaf people are the most blunt and honest type of people out there. That it's embedded into ASL to simply be more direct about everything.
