Tracing each color in Adobe illustrator with curves and lines to make shapes. It's not hard, but it's very time consuming to make them detailed.
For example,

As for this, I'm a little more worried about. With her laying down like that with so few cloths/colors I'd imagine i'd turn out funny. The process for making this style of image is not a modification of an existing image, its a full on recreation of a new one.

I just sorta wing it in photoshop with the pen tool, that's 90% of what I do with these besides zooming in nice and doing a little fine brush work. I'll include a pic that's pretty representative of how I do things in the next post. If you give me a little time I can put together a crappy infographic explaining things.

I basically trace with the pen tool around what I want to be the solid shape in the end. Once I have my shape I make sure to name the path so it doesn't get deleted with the next work path gets made, and I convert it to a selection and fill it with the most representative color in that shape. A lot of people do gradients with certain shapes that would go well with them.
I go around and do that for everything that needs it and try not to miss anything, and eventually it works out. I can't speak for anyone who does these but myself, they might have a totally different method but that's what works the easiest and most efficiently for me.
Just make sure to keep up with naming layers as you make them or you'll end up with 25+ unnamed layers that you need to order and will need to toggle visibility for a lot.

You really shouldn't trace it like this. You're ending up with a lot of wobbly and uneven lines.
Something I've found to work pretty well is using Flash to trace and fill, more or less is pretty simple.
Line tool -> Selection tool -> Bend line -> Repeat and Fill.
