It's implied that it was the grief of losing her husband that made her crazy in the first place and that Nihilego just amplified it, so I'd say that she deserves some blame but not the karmic smackdown she got.

Mohn was definitely keeping her in check. Nihilego's poisons simply remove your inhibitions, meaning that lisamine was always a bad person deep down, by she never really showed it until she was infected.

Mega Evolution isn't so much "evolution", as it is the Pokemon being pushed beyond its limits.

It has far more in common with Shonen transformations like Super Saiyans. The fact that Pokemon are apparently hurt and/or driven mad by the power furthers the parallel.
