How does the star rank up system work

Appmedia http://appmedia.jp/magireco/939238 recommends either main story battle 2-4-3 or 3-5-(2 to 5) depending whether you want a chance of sheep horn or rooftop key as a rare drop. If you only care about the letters, 2-4-3 costs 1 AP less for same amount of enemies. Unknown if they have equal drop rate or not.
Power level is calculated by how many megucas are there + their levels, or something like that. So if there's only one meguca, it isn't going to appear as a powerful foe, regardless of level.
There is no "star rank up", a 1* character has various level caps that can lead to them being level 60, where as a 5* has various level caps that end at level 90, although you can use grails to increase a character's Maximum level cap of any star level to 100. That being said a level 100 1* won't quite compare to a level 100 5*, but that seems to be the same here as well.
Fate/GO's ascension system is comparatively a little better I'd say, though. The characters don't revert to level 1, and they gain a new, generally more complex sprite along with it. Where as everything in maguca stays pretty static except for the basic card art, and even the final card arts are slightly underwhelming for a number of the characters. Madoka's especially strikes me as odd, as the 4* art seems far more... dedicated? or something, than the 5* art which is rather simplistic and cutesy.
But this is just opinions and all that.

Do quests get harder when you repeat them? Tried to repeat the Iroha solo quest since I failed one of the stars, I think the condition was that she fell below 25% health, and on the second try the witch was doing much more damage.

The implication seems to be more that she gave up on liking him after she overheard his confession to someone else be accepted. Since the content of her wish was "The courage to confess to someone I like", nothing here is wrong. It just means the wish should still technically be active.

Kako (Sci-Fi Bookworm) has a Connect that heals and cures ailments. Her Magia hits only one enemy, but gives regen to the party for some turns. It seeks to minimize overall harm, by dissuading the infliction of harm, and by aiding those who have been subject to harm. Justice is not simply repaying harm in kind. That being said, justice is subjective, however, equating revenge and justice neglects the element of restitution.

I agree with your statement that it indeed doesn't make him objectively righteous, and in fact he isn't. But I would go a step further and say such a thing couldn't be labelled as justice either. A prerequisite for Justice, to separate it from the revenge as stated earlier, would be that objective moral righteousness. Thus the difference, in my opinion I suppose, is that Justice isn't a relative concept, but closer to an ideal, where as revenge is more of an action. In that sense an act of Revenge can be Just, but isn't necessarily, or even usually so. But to call them the same thing is greatly different from the intended meaning and weight of the words.
tl;dr: Being autistic about semantics
Perhaps a more relevant example in this context would be Martin Luther King Junior. Can his actions really be considered revenge in any form? He advocated against harm and did not aim to bring whites down to the blacks' level in terms of social inequality, merely ensure that blacks received equal rights under law, which was his view of justice.

I disagree. There are very clear rights and wrongs that even proto civilizations acknowledged and followed. If you were to agree to something as universal as "complete and total genocide of humanity is wrong, and everyone who isn't significantly mentally impaired would agree to this", then you must also admit that there are at least some objective morals, and thus an objective morality.
If you would otherwise state that a complete and total genocide of humanity would not be wrong, then I'd have to question you over the statement.
"First" is not "purest." In many cases the criminal lacks the ability to "unharm" the victim; the law can do so, to a limited extent, but the means of restitution by law are more capable than a single person. The fundamental difference in that payback is that revenge is "because I was harmed" and justice is "to prevent further harm." Revenge can fulfill that condition, because there can be overlap between revenge and justice, but it is personal desire versus a system for the greater good of all.
That there is no room for compromise is also not necessarily true. Justice need not be inflexible-- this leads to oppressiveness. Recognition of subjectivity can be an element of justice, precisely because justice itself is subjective.
We live in a world of cause and effect.

Set them as your leader and run their side story missions repeatedly. Running a side story mission with the appropriate meguca in your party gives them an Episode bonus that is multiplicative with the Leader bonus. I haven't recorded which AP level of side story is most efficient (8, 12, or 16), so please post how many Episode points you got for each after running them all.

From any viewpoint you take, complete extermination of the entirety of humanity would be wrong. Justice as a system seeks to minimize total harm in the system; one can kill out of revenge with the justification that the criminal will no longer inflict harm unto others, but again, revenge and justice aren't entirely divorced. Still, equating justice with systematized revenge is overly reductionist, particularly when you distill it down to simply a "transaction."
Again, the two concepts are not unrelated, but they are not equal, either.

Definitely double charge. I'm quite sure unbuffed blast discs do the least single target damage out of three types. Blast chains do offset this as you get stacking damage increase.
