How long since you repotted it?

Chances are it's ready for a new home. Pull it out of that one and it'll probobly look like pic related, meaning its totally fucking rootbound.

Break up the dirt as much as you can and repot it in something larger. Also, make fucking sure you get some actual succulent potting soil, not just regular potting soil. They sell it everywhere, including major hardware stores.

If you can't get any, you can use regular potting soil, but make sure to mix it with a shit ton of sand and rocks.

But most importantly, repot it. That thing looks like it's probably been in the same pot since you received it.

It looks like it needs to be fed or repotted. If you've been feeding it regularly with a cactus/succulent feed mix (maybe once a month), then probably it's trying to outgrow it's pot and can't, so you should get a new pot for it. I hope you water it less than once a week.

Try a slightly deeper pot? Cacti have taproots, if I remember correctly.

Also, they really don't need to be watered very much, but when they do, they should be watered a lot (think of how much it rains when it does rain in deserts). If your pot has holes in the bottom, you should water until it drips from the bottom.

You should wait until the soil is almost completely dry between waterings.

This is actually a worse idea for drainage than no pebbles. The water tends to get stuck between the dirt and pebbles because of how water viscosity works when transferring mediums.
