It's possible that 40(?) years in a transporter loop caused his brain to shut down, and even if it didn't, you try keeping track of time without clocks or a sun or anything.
There's two pieces of evidence, one in TNG and one in ENT, that standard transporter use preserves the self and the conscious, and one piece of evidence, in TNG, that it does not preserve consciousness.
