Power armor is at the level of protection that the "shot trap" problem boob armor presents is irrelevant, as the things that can realistically penetrate power armor are things that tend to ignore shot traps (like plasma weapons).

The Sisters are the militant arm of the Ecclessiarchy, an organization that has near-bottomless wealth it can draw upon. The Sisters are an all-female force because of a legal loophole that nobody has the balls to close, so the fact that they're a female force is a big deal, both to them and the Ecclessiarchy.
They are designed to be cult-busters, bodyguards for Priests and other Ministrorum officials, protectors of Imperial shrines and relics, and to fight the Ecclessiarchys foes on the battlefield. As such, they dress for the part - they have incredibly ornate weapons and armor that practically drip with symbolism and icons of Imperial faith, custom-made specifically for them on the dime of the Ecclessiarchy.
So, why WOULDN'T an all-female, hyper-aggressive cadre of elite soldiers who are designed to be walking symbols of Imperial faith and power, who take special pride in their femininity, emphasize their femininity when it does not impede practical function in a meaningful way?
Sisters don't do subtle, anon. It's completely against everything they stand for.

I'd assume living things inside their body are along for the ride, just like their clothes and gear travel with them. Although the idea of somebody planeswalking away leaving behind a briefly floating cloud of lice and intestinal bacteria is amusing.
I the baby dies not from the act of moving from one plane to another, but from being exposed to the Blind Eternities. A planeswalker spark is required to survive them.

From your examples, though, it seems clear to me that these wizards were not mad but were engaged in complex protection rituals. Twenty-three is a mystic number by the law of fives, and blood (especially to a healer) is a powerful magic reservoir. If Seer Tarthryn stared too closely into the flames, it was because he was watching for something coming in them. Likewise Vallan, with his ritual walk, knew that an unseen enemy followed close behind.
A spell is not a mindless thing. If we were to take two men, and cut their brains in half, and somehow heal them so each had the half of the other - would you say that they would be sane? A mage does something of that every day, and the powers he use grow hungry for more of their host's mind.
I have magic inherently have dangerous effects if the ritual fails or isn't properly constrained. This includes mutations like horns or goat's eyes, and stuff like losing your shadow or being followed by vermin. It also includes various mental abnormalities and compulsions, like being unable to cross running water, being unable to look at your own reflection, etc. In my setting, powerful and long lived occultists are crazy mutated creatures who hide from the light of day, but who are sought out for their extensive knowledge
By writing magic in a psychological context and having the practice of magic and it's consequence be analogous to drug use and various forms of psychosis.
Luckily at one point in my past, I was a meth head Neo Nazi who developed amphetamine psychosis and believed that his consciousness had ascended through Luciferian ritual meditation, spiritual channeling and concentration techniques and was privy to lost knowledge of ancient magical power, so I just write based off of those experiences.

what is bad is not being able to contribute to the campaign. if you're a pampered, squishy noble and the campaign takes place in the wilderness and dungeons/caves, you're essentially dead weight and can just watch how the other characters do stuff. you're just an observer.

You really can't. The mechanics don't really support it and you can't RP by yourself without pissing off the rest of the group. Build a character around being able to participate in the group as a whole and just make it a sideline hobby for your character to be mentioned in passing and maybe used for a bit of world-building, but not much else.
