The "world", of Exorm is twisted and sickening, with cannibals devouring the flesh of stillborn young, twisted caricatures of life roaming through streets woven out of flesh and bone instead of brick. Madness and Corruption would be around every corner, and the players wouldn't be fighting the horror, so much as they would trying to survive it.

Line troops in most D&D settings are pretty low level anyway, with minimal training and shitty weapons. Which honestly isn't far off the archaic reality (as much as realism matters in D&D, which isn't much) of peasants being taken on as soldiers for a few seasons, whether for pay or by requirement.

It is ENTIRELY dependent on how well you handle it, and you shouldn't try it with a group that you haven't already been playing with for years because it's important that the initial premise be able to hook them but even more important that they enjoy what you really have planned.

Like streets moving around, neighbours disappearing etc...

Key part: NPCs should be completely oblivious to this
You can make it so they are involved, but you have to always keep up the eerie, unsettling feeling that your characters are going insane

There's a Chronicles of Darkness gameline, Geist, the Sin Eater which is entirely about people who died, then were resurrected by engaging in a pact with a Geist (think powerful Ghost returned from the underworld).

Understandably, it's quite focused on death.
However the approach it takes is quite variable, and you can even go full Día de los Muertos with it.

I liked Pawns of Chaos (set on a Chaos controlled planet with war as a mere backdrop), Eye of Terror (buddy road trip into the Eye of Terror) and Enforcer (trilogy of books with no war nor heretics, just Arbites police action and political play).

Other fun options are Eisenhorn trilogy (with possibly Ravenor to boot) and Farseer (two Eldar walk into a bar to hire a rogue trader to take them into the Eye of Terror).

Pretty routine empathic parasite cleanup, really. They can get out of hand in timelines where the Time Lords didn't already exterminate them, but if you know what you are doing they are pretty harmless.
