I was literally 10 at the time, had no idea what I was doing and played a rogue.Honestly I can't remember anything about the session other then that. Also there was like 10 fucking players and combat dragged on forever. Looking back I had no idea how I enjoyed it enough to stick with it for 15-odd years.

Depends on the setting and system.
The vast, vast majority of fantasy RPGs (even the ones which claim a degree of authenticity/simulation or show scorn towards narrative/metagame stuff) basically rely on genre conventions to make fighting a dragon work. The dragon lands and fights the heroes because that's what dragons do, as it were.
Some games go for 'realistic' dragons that don't fly or put some limitation on them to explain why exactly the dragon isn't flying around all over the place, completely out of range of the PC's and incinerating them from that distance, but they're a rare breed.

In some sci-fi settings it's not uncommon for extended lifespans to be a thing via various means suchs as augmentations or full body prosthestics. That said, wouldn't it make sense to have characters who are greater that 50 years old or at least a full hundred + years old themselves?

That's it. I'm sick of all this "Mattel Toy" bullshit that's going on in the d20 system right now. M-16's deserve much better than that. Much, much better than that.
I should know what I'm talking about. I myself bought a M-16 at a gun show for $1,350 (That's about 1100 5.56x45mm rounds) and have been practicing with it for almost 2 years now. Even solid slabs of steel are perforated by my M-16.
M-16's are built from the best composits and aliumnium and forged up to a million times to produce the best rifle known to mankind.
M-16's are thrice as powerful than AK-74's and thrice as accruate too. Anything that can be shot by a AK-74 an M-16 can do better. I'm pretty sure a M-16 can penetrate through a knight wearing full plate with just a single trigger pull.
Ever wonder why the Soviets never invaded the United States? That's right thay were too scared of the U.S. soldiers and their Black Rifles. Even in the Vietnam War the North Vietnamese targeted the soldiers with M-16's first because their killing power was feared and respected.
So what am I saying? M-16's are simply the best rifle the world has ever seen, and thus, require better stats in the d20 system. Here is the stat block I propose for M-16's.

Rate of fire 900 rounds per minute
Magazine size 20,30 or 100 rounds
range 440 yards
bullet speed 3,282 feet per second

I myself have found that only things specifically tuned for only combat have actually enjoyable combat and upgrade loops. Shit like necromunda and kingdom death.

4e is considered by many to have the most fun combat, as long as you are into the grid based tactics thing.
Strike! is an extreme streamlining of 4e's mechanics, taking some inspiration from other sources as well (XCOM for the cover system, for example).
I'd recommend checking them. They are by no means perfect, but the combat is pretty fun.
Roleplaying is much more fun than combat. Sure you might kill a few beholders in your tabletop career but you'll always remember that rat you kicked like a soccer ball at level 1.

im running a game set in a gothic horror setting. there are two leaders here, one is a vampire lord who is objectively less evil and one is a lich lord who seeks ultimate power.
i really need a reason for why does the vampire lord have a vendetta and want to kill the lich so bad.

Vampire Lord: he has been the ruler the longest, well over 800+ years, he was a general before he became a lord and lead his soldiers to killing the previous emperor. everyone was originally vampire like him.
currently all his resources go to the war against his Lich enemy, he is so focused on fighting the lich that he neglects his own people.
Lich: he came into power not too long ago, roughly 150 fish, he sought power of course, and did literally anything it took to achieve this. now that he has near immortality he's researching a way to basically harvest human souls on a mass scale to enact some sort of super spell.
Currently he's locked away in his necropolis commanding a whole army of undead to capture or kill various citizens, as well as fight the vampire lords army. but with every dead soldier his army grows.
the reason people haven't entirely left his domain even with all the murder is because its unsafe to leave town walls or you might get slaughtered by the armies of undead. its still unsafe within the walls, but at least theres protective wards helping keep the undead away.
