I've never seen a game fall in to physical confrontation, like that. In my 20 years of running games, I've seen people get mad and leave, I've even seen people cry, but I've never seen someone shove or hit someone.

At this point it really does not matter who was in the wrong. All that matters is that asshole raged and shoved the other. At that point the only thing to discuss is permabanning that asshole from the table. You do not result in physical confrontation, no matter how much the other guy was being a douche.
If a player did that to me, I can guarantee you he would never play at my table again. His character gets a humiliating death or fate, and the rest of us move on.
I don't even know the context here, but like I said, that doesn't matter when some insecure manchild is shoving people around.

Something I've noticed is that in D&D and Pathfinder a number of magical creatures will behave in ways that very wildly from how they do in folklore.
3 I know for certain are Elves, Kobolds, and hobgoblins. In the aforementioned games Elves are these noble, somewhat stuck-up creatures with an etherial air to them, while kobolds and hobgoblins are malicious and spiteful monsters that, aside from a few special snowflakes, tend to be violent assholes.
But in folklore Elves are magical yes, but also random as fuck, they were just as likely to help travelers as they were to; conjure up an anus on their forehead so they could shove a banana into it all while singing a merry song. Meanwhile you have House Kobolds and Mine Kobolds, the latter being vicious and evil creatures, while the former is friendly and helpful. And Hobgoblins, while being described as being as ugly as goblins, tended to act more like Hobs. which according to Wikipedia

First time DM here running HotDQ and RoT. Are you running a module or your own homebrew? Personally I put in a couple hours a week in and some people will say thats loads for a module. But I am running it for 9 people! (7 most weeks though) so i need to balance everything quite a bit. I do add things in to the campaign every now and then, like interesting encounters or story elements that i think make more sense but more often then not if its added in its in response to rp so I just improv things on the fly. Mybplayers are having fun and enjoying the campaign so i must be doing my job okay.

First of all that's Dragonlance which is only somewhat D&D related, and it's good because it didn't have to be anything other than an awesome book cover, possibly based off of straight mythology and history
Nowadays there's a shitload of conventions that come with the standard "elf" or "human" or "dwarf" that it would alienate too many if they didn't have the most bog standard art possible
Because old art had inspirations from the era; heavy metal, renaissance fairs, all those fun things. Also the art used real people in poses as references
Same goes for current, but it's got anime, cartoons, digital drawing, etc etc. basically it's become so abstract in making the art that it's lost all soul
