It has a lot more to do with diet than you think. Even though I brush twice a day nowaways, I've gone months without brushing and had no repercussions, because my diet is very low in sugar.

Teeth issues can run along with diet and your tooth microbiome - there are people who can get away with rarely or even never brushing, and people who brush, floss, and use mouth wash twice a day, who still have fucked up-looking teeth. Certain kinds of bacteria and acidic/abrasive/sugary foods have bad effects on teeth, and if you don't have those consistently present, you can have relatively healthy teeth with low maintenance. That's why humanity didn't brush its teeth for thousands of years before now, but everybody wasn't constantly doubled over in dental pain. Management of how 'acidic' your mouth has become, and therefore how cavity-prone, is becoming a newly, better understood thing in dental medicine, and if you're a heavy coffee/soda drinker, I'd recommend looking up xylotiol gum and/or high pH toothpaste. It makes it so that constantly drinking that sort of stuff doesn't fuck you over in the long run.

You should still go to the dentist's for a cleaning twice a year, though. Most American workplaces provide base-level dental insurance for like $10-15 a month, which entitles you to those cleanings for free. Almost nobody has teeth so perfect that they couldn't benefit from a thorough scraping/polishing. Humans are naturally capable of keeping healthy teeth, but even under ideal circumstances, our bodies aren't built to last with healthy, white teeth for more than ~50 years.

But that's not true. Sugar isn't really a problem. Sugar dissolves quite quickly in saliva.

The real problem is sticky starches (such as potatoes) and carbonated drinks.

Sticky starches only come off your teeth by brushing. Bacteria love sticky starches, and eventually form plaques. And carbonated drinks can strip away enamel faster than it can be remineralized if you have too much.

Try eating a potato and then washing it down with coke. Then go straight to bed. I guarantee you'll have rotten gnashers within a year.

Sugar substitutes (sugar alcohols) can't be metabolized by the bacteria in your mouth and therefore don't cause cavities. Even without sugar, the acid in soda can still erode your enamel, though.

People were dying at a much younger age so they didn't have as much time to develop long term dental problems. They also were usually missing teeth or had dentures as early as their 20s and 30s. Pro tip: Ask your dentist for a special gel which can reduce a pain by syringe injection and you will not fear going there anymore.

I used to work as a dental receptionist, if a dentist tells you you have periodontal disease, he's full of shit. A periodontal cleaning, which is supposed to happen every 4-6 months, costs like $400. Most of what they're selling is bullshit. Brushing your teeth is good, but I'm sure baking soda and other home methods are just as effective as jew paste.
