I've seen it thousands of times.
A new guy comes in, it's his first job and the position really doesn't cater to people like that, they are nervous, have no situational awareness of what actually needs to be done.

In the end they get dropped because they fail to provide sufficient common sense and reasoning to do what they are expected to do.
It's not always that they are incapable it's just that they don't function in the intended capacity.
