Go get the surgery
Then pay them $15 a month for 7 years
After 7 years it no longer impacts your credit, so you can just drop it and ignore the debt collectors.

Hospitals have to take whatever you give them as "good faith" payments on medical debt. If you don't have insurance they'll often also drop the cost of the procedure by 85% because that's what they normally walk away with after dealing with insurance companies anyways.
