Just use a blindfold and jump from a building. You don't have to jump immediately.

Go up there, use the blindfold and drink alcohol, listen to music, eat your last meal, when you are ready you can simply jump.

You can't get scared of heights if you don't see them. This is a foolproof way to suicide. There are reports about people falling from a fucking chair and dying. If you do it from a sufficient height you WILL die immediately and painlessly.
