Technology is much more advanced today; I can imagine that there are several ways to inflict more pain and suffering.

As a first step; you'd probably want to isolate the blood supply of the brain onto a heart/lung machine; that way you can keep the head alive and functioning (and aware) no matter what you do to the rest of the body.

No blessed shock cutting off the pain; and someone could be fully aware and awake and feel their body die.
