Well... As far as I've done things you can go at it in two possible ways.

1a) SAVE OFTEN and keep backups in the same PSD file

1) if the area covered is very thin, mostly evenly lit, you can use WISELY the clone-stamp tool to extend skin over the covered areas. Be careful to calibrate correctly the opacity.

1b) Take some area of skin which seems reasonably similar, lasso tool, copy and paste. Then use a 0% eraser for the edges and Warp function (in Edit) to change the shape, then again eraser and so on... Multiple applications can be a possibility for hair (for example)

2) Find a suitable donor. This is by far the easiest and the most difficult. When choosing a donor you have to take a few things in consideration:

a) lighting - even if it's not a perfect match, consider that same-starting conditions can be critical to achieve a good result. Of course the instruments are either an Exposure Level or just applying Image -> Exposure to the level (the difference is in the first case it affects every Level beneath while in the second it affects only the level you're in but can't be taken back later on)

b) skin-tone Even the most perfect lighting can't do much if the skin tone is completely different. You can tweak more or less in some instances but it's difficult to reach realism if you don't go the extra mile.

c) Angle - Depending on the area you're messing with the angle is extremely important otherwise the pic isn't credible at all.

d) QUALITY of the original pic - This is difficult. You can't upgrade quality, that's a movie thing. So you have to downgrade the donor more often than not. There are a few possible techniques for this
d1) Paint daubs (Artistic filter)
d2) Blur (usually more than one application)
d3) Sharpen (usually also more than one application)
d4) Burn tool (applied to areas that must match pre-existing overexposure)

And definitely more, more more... Take a pic and see what works.

And of course, you can combine multiple of this to different extents

One final thought. Your best friend is the Eraser. Your worst enemy is bad pictures.

These are the opacity level I use for the Eraser
85% cloth or edge of the skin
45% - 75% hair
0% inside an area
Again, not necessarily always true. Many times I'll do differently, but you can start with these I think
