How does that even work? Piston engines need flammable fuel to function.

a normal piston engine has fuel and air mixed to make a flammable substance, and then the spark plug ignites the substance, forcing the piston back down, which in turn fires another piston up via the crank shaft.

the way his works is that pressurized air goes in through an "intake" (in this case he was blowing into it) and by spinning the crank under the piston; the wooden dowel on top pushes open the spring mechanism (where your spark plug would usually be) that allows a blast of air into the cylinder, thus pushing the piston down, spinning the crank and allows the piston to reset, causing the valve to open again etc...
