Honestly, for a hell of a lot of trans people, some sort of psychiatric treatment that they can keep to themselves would probably be preferable, but it just doesn't work. Transition is the only treatment that's been shown to be effective, and considering the stigma against doing it, you know the medical community's really given everything else a red hot go.
So if your sibling is set on this, just know they've likely considered all other possibilities already. It's their body after all, and as freaky as it might be to witness something like this, it's even freakier to go through first hand.

They always look a little weird.

Gender dysphoria is a neuropsychological condition, widely believed to be a fuckup in the part of your brain that keeps the mental map of your body. For example, trans women almost never have phantom limb syndrome after gender reassignment surgery (peenor removal), while men who lose their dicks in accidents do.

Medical transition is the only known treatment for gender dysphoria. Think of it like ADHD, which is also a neuropsychological condition. You can treat ADHD, but you can't cure it. Therapy and antidepressants/antipsychotics/etc will not cure someone's ADHD, nor can you fight it through the power of loving your body no matter what society tells you. Gender dysphoria is the same way, you can treat it but you can't cure it.

Believe me, I wish it didn't work that way. If I could take a magic pill to not be trans, I would do it in a heartbeat. My life is made infinitely shittier and harder by my dysphoria and need to transition. My family will probably disown me. My girlfriend may not be able to stay with me, she's very supportive but also very much a lesbian. It sucks, but I'm going to transition anyways because as shitty as it's going to be, dysphoria is shittier.

And thank you for being respectful. It's great that you're so concerned for your sister (now your brother), that you're concerned with his safety first even if you don't really understand. It's a completely normal response to not want your sibling or family to be trans, it's a hard process socially.

Even if you don't end up liking his decision, he's going to need a lot of support during this process. Suicide rates are astronomically higher in trans folks in unsupportive environments, with families and friends who refuse to accept the transition, call them the right pronouns etc. He's going to need someone on his side.

people see physical illness or hear someone has a neurological disorder and there's a level of sympathy usually and an acceptance that it's out of the person's control... whereas people have a senseless reaction towards mental illness and act like it's a shortcoming the individual has control over... or like they can somehow "fix" it by treating the person a certain way...

i've seen being trans compared to schizophrenic delusions enough times to think a reclassification based on evidence would probably make a difference in the long run...
