Most hearing damage is cumulative, and it can sneak up on you later in life, or it seems not that bad when it first happens but then it becomes worse on it's own as you get older.
One .38 Special without earpro isn't good, a .223 or few indoors is pretty bad, but it's all a shitload better than shooting a brick of .22LR in an afternoon because that tinnitus is gonna come crashing through your door like the Kool-Aid Man.
