Worst are in poorer or less populated places with harsh winters obviously, North Dakota, Maine, etc

Arkansas has noticeably horrendous roads, not sure why exactly. I have been there and they are just awful, I also remember my dad, who's a truck driver and knows all the roads nation-wide, saying they were so bad in Arkansas that he refused to vote for Bill Clinton because the man clearly couldn't run shit. (Clinton was Governor of Arkansas before becoming President)

Being from Southern California it doesn't seem too bad to me either. I have been to China and Mexico and the driving there was more hectic. However, in other parts of the US I have noticed that the driving is different and more orderly from what I am used to in SoCal.

Tunneling is less than optimal in many parts of the Netherlands.. If you dig a few feet in this country you hit water, need to drain more, gotta make sure your tunnel segments don't float up, the settlement of the soil etc. A bridge is probably less intensive, especially because it's a light bridge that doesn't have to bear automobiles and such.

Double-deckers are for moving masses of people around a comparatively small space. Aerodynamics aren't much of a concern compared to stop-start traffic with eighty, ninety people on board.

As for height restrictions: generally there's a guaranteed overhead clearance for road-going vehicles (can't build bridges below X height), which you can plan a bus route around anyway.

Depends. Most cities have their specificities.

Paris is entirely gridlocked all the time and if you spend more than a second to start after a red light you are as good as dead, Lyon has chaotic highways and rings around it but has outstanding traffic otherwise, Marseille has shitty and dangerous drivers. The city I'm in currently in (Grenoble) could been summed up as "you'll get faster by foot anyway".

I think that one probably happens everywhere on Earth, though. The favourite for lots of people here is to race through traffic as fast as you can so that you can stop at the red light 100 metres up the road with everybody you just passed anyway. Then they try to speed off to get away from traffic and get stopped at another red light a few hundred metres down the road again.

Because it's standardized across all our other roads, including smaller local-highway ones that might have two lanes each way and no divider, or one traffic lane each way sharing a central turn/merge lane. On such roads, the yellow line is the hard split.
On long two-lane stretches, the yellow line also indicates which direction can pass; a double yellow means no passing, a dashed yellow means either can pass if safe, and a solid yellow alongside a dashed means that only the lane on the dashed side may pass.

Additionally, the close white dashes indicate an exit or merge lane that will end or diverge soon.
